Create Report: Cases Created/% Escalated

Hello,

Can someone please help me with how I'd create a particular report I need for my organization?

I am looking to make a report that shows a 6 month trend graph of:

1) Total # of Cases Created Per Month

2) % of These Cases that were escalated/moved to a particular team in a month

3) Total # of Cases Resolved by a particular team in a month
