How to get/retrieve Text value and show as Option-set ?

Hello,

Hope you know Address Entity cannot be link to any other entity except contacts/account. So i created custom entity  as"Custom Address".

On this entity, have created a text value as Address Name and a customer will have multiple addresses to enter Address Name.

Now, on the lead entity i want to show this Address Name text value as Option set, how to retrieve text value and show as Optionset ?

Example : Multiple Address Name (GE Corp, GE India, GE China)etc.

Need a filter or optionset to pull from Customer Address entity and Display in Lead entity.

Please help.
