HTML Web Resource in Address entity form- Dynamics 365

Hi All,

I am facing issue after upgrading to Dynamics 365.  There is HTML web resource on Address entity form which is working fine in CRM 2016.  But after upgrade, address form not loading HTML web resource, it's loading empty iframe without content.   Noticed error 404 not found for below webresources.js file in browser developer tool.

Any idea what could be the issue. I can successfully add the same web resource on other forms.
