Pass client record to It's Phone call record

Hi all,

I have a client form as shown below:

When I click on add phone call as shown below:

When I click on add phone call, I have a form as shown below:

What I need here, I need client's phone number and other client information on this form(auto populated). When I click add phone button. How can I achieve this task?
