AdxStudio Service Request galery

Hi all,

I installed a website and include Information Request from the Service Request Library. I use Adx 7.x with Dynamics 365 on premise. When a user login and try to navigate to 311/Information Request. There is a page with the sentence Information Request but have no form to fill out.

Did I miss something ?
