I need to create record on the Home page

Hi all

I need to create record on the Home page from my custom button for my custom entity. I am following this blog https://lakshmanindian.wordpress.com/2012/10/11/create-record-using-jscript-in-microsoft-dynamics-crm-2011/ . I am stuck on a point Where should I add these two files "To perform an ODATA call, we need to add jquery1.4.1.min and json2 files in your Form Properties. You can get those files from sdk."  on form properties? I add these two files on form properties. but the code giving me this error

 What is wrong? Please help me.
