Business process flow for multi entities

Hello,

I am using CRM Dynamics 2016 on-premise

I've created a BPF which starts from incident and continues to a different entity (one time for custom and one time for phone activity).

this BPF is working as expected only 1 time for each server.

starting the 2nd run of the BPF i fail to see the BPF line once i go into the 2nd entity.

I tried that on a different organization on 2015 and it worked fine

any ideas? is this a bug?
