Dialog to create a case, Account, Opportunity , etc

I have a few questions regarding the dialog process. I am looking to create a dialog which first determines the type of call, sales, technical support, etc that will be selected via a radio button. Based on that selection I would like to ask questions and then create or build a record, a case if it is technical support related call, a lead, or an Account , Contact and Opportunity if it is regarding new service, or whatever else I can think of!

The first issue I see is I am unable to launch a dialog which is not already tied to an entity. I tried starting with dialog related to the a phone call entity. I could create the phone call and start to collect the data and maybe run child workflows based on the responses. I cant seem to be able to launch the dialog without a current phone call being saved first?

Is there a way to launch a dialog that is not related to an already created or related record?

Any insight into how to get this started?
