Appointment not appearing in Outlook

When entering an appointment on the mobile web version of Microsoft Dynamics v 2015, the appointment shows in CRM but doesn't get synchronized to Outlook. Any appointment set through the standard CRM web screen, syncs without any problems. I've been able to recreate the issue on three different CRM's  - hosted, on-prem and cloud.   

Has anyone experienced this and have a resolution?
