Custom button not reflected on ribbon in CRM 2015

Hi,

I have added custom button on Order(SalesOrder entity ) Form using ribbon workbench but it's not reflected on ribbon.

When I open Order record it is not available on the form. (using CRM 2015 On-Premise)

Need pointers urgently.
