Outlook silent configuration fails due to missing credentials

I'm attempting to deploy the CRM Outlook add-in and silently configure it using the CRM for Outlook Configuration Wizard and the Default_Client_Config.xml however the configuration fails due to missing credentials (See error log below).  If I open Outlook the configuration wizard pops up and using CRM Online as my Server URL I can test the connection, be prompted for and enter credentials, and the configuration works. Unfortunately, I need to do a silent installation because I cannot expect that my users will properly navigate through a manual configuration.  

I've tried making sure credentials are saved in the Windows Credential Manager, also removing the credentials from there as well.  Also reinstalled Windows Live ID (suggestion from microsoft KB) which did not seem to work.

Any help or suggestions would be much appreciated!
